1. Take the digital assessment.

In about five minutes online, tell us about your loved one's health, home, routines, and the worries that keep you up at night. Create a password and come back anytime. After you begin (and complete payment), you can schedule a live one-on-one session with us as part of your assessment.

2. Receive your personalized care plan.

We build a plan around your family. It covers what to do first, who can help, and what to watch for down the road, with trusted resources and tools that help your loved one stay safe and independent at home.

3. Use your family portal.

Your plan lives in your private family portal, along with appointments, products, services, and notes. You decide which family members can see it, so siblings near and far can pitch in without endless group texts.

4. Start strong, then choose your membership.

Your $89 assessment includes a 90-day start period to put the plan into action with us. After that, choose the membership tier that fits your family, based on how much portal storage you need and how often you'd like to talk with us. Membership starts as low as $25 a month, and you can change tiers as your needs change.
