How to Use This Book
The first step in utilizing this book is to read it through and make sure you
comprehend each section before sitting down with your loved one. Not all
sections may be relevant to your situation on day one, but you may find them
helpful further into your senior care planning journey.
The first section contains reference documents that need to be filled in with
relevant medical, financial, and legacy information. Please be aware to update and
save the Updated as of: tab on each entry to ensure your information is current.
In the second section, you’ll find resources regarding legal documents such as
Power of Attorney, Health Proxy Guardianship, and an example Letter of
Competency. Each of these comes with an information sheet explaining what
each title means and their responsibilities.
The final section is more of a collection of reference material such as lists of
senior services, dementia references, specific difficult-conversation guides,
senior living options, self-assessments, and useful definitions.
After you’ve gone through this book, it should be held onto by the primary
Power of Attorney. That way, they can reference it whenever there’s a question
about your loved one’s wishes.
